How to tell whether it really failed

Before you worry, know the normal timeline: transplanted hair typically sheds within 2–6 weeks (shock loss), the follicles rest for a few months, new growth usually starts around months 3–4, and the final result takes 12–18 months. Judging before 12 months is too early.

A transplant that has truly failed shows clear signs at 12 months or later: sparse, uneven growth; an unnatural hairline angle; visible scarring in the donor area; or much less density than you were told to expect. If you see these, you may have a real problem worth a second opinion.

Common reasons transplants fail

A common cause is poor technique: rough graft handling, wrong angles or an over-harvested donor area. Before any procedure, ask who performs each step, and get the answer in writing.

• Poor graft handling: grafts kept outside the body too long, or in the wrong solution, can die before they're implanted
• The wrong technique for the hair type: for example, FUE on very curly hair without adjusted extraction
• A graft count that doesn't match: 5,000 grafts promised, far fewer implanted
• Poor hairline design: too low, too straight, or an unnatural pattern
• Aftercare not followed

Revision: often the next step

A revision (corrective) procedure can often improve a failed result. It's more complex than a first transplant and needs a team experienced with scar tissue and previously transplanted areas.

How to choose a clinic for a revision

For a revision, experience matters even more. Look for: photos of revision cases specifically, an honest assessment of what's achievable from your photos, and a clear explanation of the approach and who performs each step.

Send photos of your current result to more than one clinic and compare the assessments. A trustworthy clinic is honest about limits rather than promising too much. At the partner clinic, previous failed transplants are quoted individually after the doctor reviews your photos.
