Hair transplant aftercare: 10 mistakes to avoid

Common aftercare mistakes after a hair transplant: sleeping position, washing, exercise, sun, and the first-week care that protects your grafts.

Why aftercare matters

Even a well-done procedure can be let down by poor aftercare. The first 10 days matter most: the grafts are anchoring into the skin and are easiest to dislodge.

Here are the most common mistakes and how to avoid them. Typical guidance — the clinic's written instructions come first.

Sleeping flat

Sleeping flat can make forehead swelling worse. Sleep with your head raised (about 45 degrees) for the first nights, with a travel pillow or stacked pillows.

Touching or picking the scabs

Tiny scabs form around each graft; that's normal healing. Picking, scratching or rubbing them can pull grafts out. Let them come off during washing (usually by days 10–14) and keep your hands off the transplanted area.

Washing too hard, or too late

The clinic does the first wash, usually the day after the procedure, and shows you how. Waiting too long lets scabs harden; washing too hard can dislodge grafts. Follow the clinic's washing routine exactly.

Exercising too early

A higher heart rate raises blood pressure in the scalp, which can cause bleeding and dislodge grafts. Typical guidance: light walking from day 1–3, the gym after about 2 weeks, swimming and sauna after about 4 weeks.

Five more to avoid

Sun: UV can damage healing skin and leave lasting discoloration. Wear a loose hat outdoors for the first 3 months.

Smoking: nicotine narrows blood vessels and reduces blood flow to the grafts. Stop ideally 2 weeks before and 2–4 weeks after.

Alcohol: it thins the blood and raises bleeding risk. None for at least 3 days before and 1 week after.

Tight hats: baseball caps and beanies can press on the grafts in the first 2 weeks. Wear only a loose, clean hat.

Skipping medications: take everything the clinic prescribes; skipping raises the risk of infection and swelling.

Panicking during shock loss

Transplanted hair typically falls out at weeks 2–6. This is expected: the follicle stays alive under the skin and new hair usually starts growing from months 3–4.
